CERTIFICATE OF ORIGIN SERVICES
Pricing for 1 certificate, per shipment (Please have 3 copies of each original form ready for processing)
– President’s Club members: Complimentary
– General Level members: $25 each
– Non-Members: $300 each
Processing Guidance

For processing, please have on hand:
– 3 printed copies of the Certificate of Origin form (accessible above) to be stamped/endorsed
– Current ID
– Any supplemental invoices (Leave a 3-inch space on your invoices for the approval stamp)
Please note: Any unfilled lines will be crossed out during processing, as they could be filled in later with additional information that will alter the form from what the notary is signing and stamping.
NOTARY SERVICES
Pricing for 1 notary
– Members: Complimentary
– Non-Members: $5 per notary
Processing Guidance
For processing, please:
– Scan/email the forms you will need notarized ahead of appointment time and email them to Operations Manager Joyce Chatman at jchatman@vegaschamber.com
– Current ID
– Any supplemental documents
